Job DescriptionUnder the direction of the Youth & Family Service Manager, this position works collaboratively with youth and their families to deliver intensive case management, education related to mental illness and emotional disturbance, and to develop coping skills primarily in community-based settings, such as homes and schools.
This position conducts routine needs assessments, completes recovery plans, assists youth and their families with accessing community resources, and initiates crisis supports when needed.This position is supported by an electronic health record and provided in collaboration with a multidisciplinary recovery team made up of psychiatrists, nurses, counselors, and family partners who have lived experience with receiving services.
