Responsibilities

• Providing primary care management, nursing support, coordinating & scheduling care for clients to ensure they maintain dignity, independence, and joyful living.
• Developing and maintaining dynamic care plans and schedules for clients.
• Leading/coaching teams of caregivers and interacting with multidisciplinary team (physiotherapists, massage therapists, doctors, etc.).
• Teaching new graduate PSWs various elements of caring for clients.
