Case Manager - Brighton Center For Recovery



Be a vital advocate at the center of patient care.

In this role, you will lead and coordinate the interdisciplinary plan of care, guiding patients and families seamlessly from admission through discharge. You’ll serve as a trusted partner to patients, providers, and leadership—ensuring care is collaborative, compassionate, and aligned with individual needs and values.



+ Monday–Friday work schedule (8am-530pm)

+ Occasional holiday coverage may be required

+ Pay Range: 23.26/hr / 23.76/hr / 25.88/hr

Responsibilities:



+ Design and coordinate comprehensive care plansacross the continuum, integrating patient and family goals, preferences, and clinical needs.
