The Clinical Director, Professional Practice & Integrated Care provides senior system-level leadership for professional practice, clinical governance, quality, education, and integrated models of care across Nucleus’ home and community-based services. The role will ensure the delivery of safe, high-quality, evidence-informed, and person-centered care aligned with Ontario Health priorities. The Clinical Director is a key architect of Nucleus’ innovative models inclusive of the Florence Nightingale nursing initiative, and One TEAM interprofessional approach. This role bridges frontline clinical practice with organizational strategy, digital innovation, system partnerships, and workforce sustainability, enabling clients to remain safely at home while reducing system pressures.
