Consult and collaborate with colleagues, clinical, medical, educational and other community services in conjunction with families/guardians to plan, coordinate, and evaluate support (such as transportation, medical appointments,respite care) using various communication methods.

Provide and/or facilitate training opportunities as identified through the Personal Plan Transitional Aged Youth, Service and/or Care Plans.

Schedule employees to ensure adequate coverage for after school programs, camps etc.

Participate in staff and community meetings. This may include participation in regional and provincial activities associated with Family Support programs.
