Become an Intake Case Manager with Kiwewin at the Grande Prairie Friendship Centre. This role focuses on providing essential case management support and resources to those facing imminent homelessness.
As part of the Kiwewin program, you will deliver short-term financial assistance and case management. Qualified candidates should possess a Human Services diploma or degree and have experience with mental illness, substance abuse, and behavioral challenges. You will also need excellent communication skills and a valid Class 5 Driver's License.
Key Responsibilities:
• Report daily to the Program Coordinator
• Assess program eligibility for incoming calls
• Provide information and referrals to participants
• Support housing sustainability for participants
• Maintain confidentiality and accurate participant files
