Job Description
Elevate hospice care as a Palliative Care Nurse Specialist. Engage in client assessments and implement comprehensive learning programs while supporting a dynamic interdisciplinary team on a casual basis.
In this role, you'll coordinate client activities and serve as a resource for nursing staff. Your responsibilities include developing education plans, evaluating care practices, and helping clients navigate care needs. Your leadership will enhance the quality of life for clients and their families in their time of need.
Key Responsibilities:
• Monitor standards of care and provide consultative support
• Direct training programs for clinical staff
• Evaluate and modify client care plans as needed
• Conduct interdisciplinary care meetings and rounds
