What you’ll do

• Conduct initial client assessments, identify problems, and establish goals and treatment plans.
• Develop client‑centered plans with clients, the inter‑professional team, and other stakeholders.
• Engage clients using a collaborative approach, build rapport and trust, and achieve mutual understanding and agreement about services.
• Maintain professional client records related to encounters and plans.
• Work collaboratively with a multidisciplinary team of government agencies, healthcare workers, and community resources.

What you bring

• Bachelor’s degree in Physiotherapy.
• Registered and in good standing with the College of Physiotherapists of Ontario.
• Willingness to travel to and from clients’ residences within a designated area.
• Valid driver’s license and access to a vehicle for travel within a geographical area.
