RAI & Restorative Lead Coordinator – Full Time

Reporting to the Director of Care.

Responsibilities

• Review interdisciplinary and comprehensive resident assessments, collecting data for quality documentation and ensuring adherence to MDS and InterRAI LTCF requirements.
• Support nursing and accounting departments.
• Manage coordination and completion of Resident Assessment, Clinical Assessment Protocols and care plans as assigned.
• Comply with MLTC regulations regarding the InterRAI LTCF process.
• Accurately code InterRAI and care plan documentation for all residents.
• Continuously update knowledge base related to data entry and computer technology.
• Chair RAP chat as required.
• Provide restorative education, support and assistance to staff and residents.
• Assess new residents for restorative needs.
