Summary of responsibilities

Reporting to the Manager, System Navigation, Geriatric Mental Health and Community Withdrawal Management, the Registered Nurse will provide support and consultation for clients with dementia or delirium, who are exhibiting responsive behaviors. Working with the client, their family/caregivers, as well as the interdisciplinary team, the nurse will offer case management, collaborative care planning and consultation to support complex transition across the continuum of care from the community to Retirement Home and assisting with the Long Term Care Transition as needed. Services will be offered in the community/home environments.
