Job Overview

Reporting to the Care Manager, the Case Manager is responsible for directing the care of clients under the home care program. Using strong assessment, organizational, and leadership skills, you will identify client needs and facilitate the interdisciplinary team with the appropriate internal and external resources required.

You will interact with the team and client/family to coordinate care planning and facilitate client and family teaching. Through the development of the care plan, you will ensure services are provided and modified on an ongoing basis or as needed.

The successful candidate will play a key role in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. Experience in providing wound care and knowledge of wound care principles and products, as well as palliative and home parenteral therapy, will be essential to your success.
