Detailed Overview

As a member of a multidisciplinary team, provides direct nursing care to patients of the Hospice Residence at assigned locations according to established policies and procedures by assessing physical, emotional, spiritual and psychosocial needs; plans, documents and implements care plans; evaluates the effectiveness of the care plan and makes recommendations for care plan changes, where necessary. Responsibilities Assesses, plans, implements and evaluates nursing care of assigned patients by assessing actual or potential problems and strengths, plans and performs interventions and evaluates outcomes. Collects information about the patient using a variety of sources such as skills of observation, communication and physical assessment to enable appropriate nursing care.
