Overview

We are looking for passionate and caring Social Workers to join our Integrated Care Solutions team. The position is part‑time, requiring 2‑3 days of availability Monday‑Friday, approximately 15 visits per week, in the Markham, Uxbridge, and Whitchurch‑Stouffville areas.

Duties and Responsibilities

• Communicate with clients, families/caregivers, service providers and other professionals to collaborate and coordinate services
• Maintain and manage a caseload in an efficient and effective manner ensuring priority code timeframes are met
• Assess the client’s adjustments to medical, emotional, psychological or social problems and the factors that affect them
• Utilize a client‑centered approach to identify, prioritize and monitor potential psychosocial problems and goals
