As part of Case Management at Unity Health Centre, the Senior Assessor Coordinator works with clients and families to ensure effective continuity of care. Key Responsibilities:
• Create individualized care plans for patients
• Monitor clients’ progress and adapt plans accordingly
• Liaise with families to address their support needs
• Work with healthcare teams to provide comprehensive care
• Support clients in maintaining independence post-discharge
Requirements:
• Bachelor of Science in Nursing (BScN)
• 1 year experience with community health care
